DFPlayer Mini - During some files player stops with error that card was removed then re-inserted

I'm loving the DFPlayer Mini, however I'm running into an odd issue. When I am playing some files (usually specific ones) the DFPlayer mini will stop abruptly (light goes out). Using the sample's DFDebugPrintDetail in my loop function while this happens is saying it is getting an error code that the SD card was removed and then immediately re-inserted:

Tried to attach a diagram of my breadboard, but it won't upload the .jpg or .png :S

It doesn't happen with every file, just some specific ones it seems and in the same place. What makes this more odd, is that I've seen the audio files work from time to time... but now they consistently seem to have issues.Card Removed!
